Does your student participate in school sports or band? Per MSHSAA guidelines, all students involved in MSHSAA-sanctioned activities—such as athletics or marching band—must have an up-to-date PRIVIT Profile on file. Each year, our school’s athletic trainer works hard to keep student records current and accurate. Please assist her in doing your part to ensure your student is eligible and ready to go on the first day of practice by following these simple steps:

  1. Log in or Register
    Visit the Warsaw PRIVIT page. If you’re new, click Register as a Parent/Guardian and create your account. If you've already signed up in past years, simply log in.

  2. Add Your Student (if applicable)
    After logging in, use the Add Member option to link your student. You’ll need basic details like their name, birth date, and gender, etc. If your student is already linked, you can proceed to the next step.

  3. Join Teams (if applicable)
    Click on your student’s name, choose Joined Teams, and select all current sports, activities, or programs they’ll participate in.

  4. Personal Details & Health Forms
    Go to Personal Details and complete every section marked with a red asterisk (*).

  5. E-Signature
    Review each form and apply your electronic signature where indicated.

  6. Turn in Medical Eligibility Form
    Students must turn in a physical copy of their Medical Eligibility Form to Athletic Trainer Lisha Douglas. This is an important step to ensure the process is finalized.

Please note: If your student received a physical at school this spring and has an active PRIVIT account, the form has already been uploaded.
